Description

American Traveler is hiring a Home Health RN with required OASIS experience and home health/hospice background for a 13-week dayshift assignment.

Details

Home health and hospice setting providing outpatient care through patient home visits
Includes wound care responsibilities
Shift schedule includes Friday, Saturday, Sunday, and Monday 8am-6pm
Also described as Monday to Friday 8am-5pm with flexibility based on patient needs
Scheduled for one weekend per month, followed by two days off
Some emergency-based call may be required
Uses HealthWyse (Careficient) EMR system
13-week assignment

Requirements

Active VT or compact RN license required
BLS certification required
Minimum 2 years of experience required
OASIS (Outcome and Assessment Information Set) experience required
Home health and hospice experience required
Must have a valid driver's license in good standing
Must be able to travel between patient homes for visits
Must be able to manage and adjust to schedule changes

Additional Information

Provides home-based nursing care including wound care and hospice services
Facility conducts its own background check in addition to standard requirements
