Description

American Traveler is hiring a Registered Nurse for a Hospice/Palliative Care community role requiring NJ or compact RN license and two years of home hospice experience.

Details

• Community-based hospice and palliative care role within a homecare setting
• Caseload includes approximately 4-5 patient visits per day plus admissions
• Schedule is AM shift, 8a-4p, every other weekend with comp days during the week
• Floating to different counties as needed based on service demand
• No on-call responsibilities

Requirements

• Active New Jersey RN license or compact license required
• BLS certification required
• Valid New Jersey driver's license required
• Minimum two years of RN experience in a home hospice environment required
• Associate's (ASN) degree or diploma with NJ RN license required
• EMR experience required with strong critical thinking and communication skills
• Three or more years of hospice nursing experience within a hospice agency preferred

Additional Information

• Performs patient assessments to determine hospice appropriateness and develops individualized plans of care
• Completes admission, discharge, and re-visit documentation with twice-daily data transfers
• Provides age-appropriate patient and family education and revises teaching plans as needed
• Manages caseload including prioritizing care needs and coordinating with the interdisciplinary team (IDT)
• Attends IDT meetings every two weeks and monthly staff meetings
• Supervises home health aides (HHAs) per established standards
• Maintains safe patient environment through infection control and safe medication management practices
• Participates in performance improvement activities and may provide orientation or mentorship to new employees
