Job Title
RN Home Health - Grand Rapids, MI (Local or Travel)
Assignment Details
Assignment Length: 13 weeks
Schedule: Monday-Friday, 8:00 AM-5:00 PM
Hours: 40 hours per week
On-Call: Every other week; triage nurse takes calls
Weekend Coverage: Weekend RN available
Productivity: 30 units
EMR: HCHB (Homecare Homebase)
Required Experience: HCHB, OASIS, and Home Health
Guaranteed Hours: No guarantee
Requirements & Qualifications
Experience: 2+ years of Home Health experience
HCHB & OASIS: Required
License: Active RN license in the practicing state
CPR Certification: Required
Driver's License: Required
Transportation: Reliable transportation and valid auto insurance required
CHPN Certification: Preferred
Education: Graduate of an accredited Associate Degree in Nursing or Nursing Diploma program required
BSN: Preferred
Required Skills
Homecare Homebase (HCHB): Required
OASIS: Required
Strong clinical judgment and patient assessment skills
Excellent communication and documentation skills
