Physical Therapist - PT Home Health, Moline, IL - Local or Travel

Contract Duration: 13 weeks

Shift/Schedule:

• Monday - Friday

• 8:00 AM - 5:00 PM

• 40 hours per week

Requirements:

• Must be licensed in IA and IL

• HCHB and CWHH experience preferred

• Must have OASIS experience

• Covering Quad City area and some outlying areas

• 36 hours guaranteed

On-Call Information:

• On-call required if requested. On-call needs are subject to change based on staff and branch needs.

Qualifications:

• HCHB, OASIS, and Home Health experience required

Skills:

• Current State License as a Physical Therapist - Required.

• Proof of current CPR certification - Required.

• Current driver's license. Must have reliable transportation and insurance - Required.

• 1-2 years experience as a Physical Therapist in an acute care or rehabilitation setting - Required.

• Previous home care experience - Preferred.

• HCHB EMR preferred.

Education:

• Bachelor of Science in Physical Therapy from an American Physical Therapy Association approved program - Required.
