Position Overview

ProTouch Staffing is seeking a qualified Physical Therapist (PT) for a Home Health contract opportunity in Sandwich, MA. This role provides physical therapy services to patients in a home-based setting while managing an assigned caseload and delivering high-quality patient care. The position requires travel throughout the assigned service area to evaluate patients, develop individualized treatment plans, and support improved mobility and functional outcomes.

• EMR: HCHB (HomeCare HomeBase) experience highly desired

Requirements

• Current Physical Therapist License in the state of Massachusetts - Required

• Current CPR Certification - Required

• Valid Driver's License with reliable transportation and active auto insurance - Required

• 1-2 years of experience as a Physical Therapist in an acute care or rehabilitation setting - Required

• Previous Home Health/Home Care experience - Preferred

• HCHB EMR experience - Preferred

Education

• Bachelor of Science in Physical Therapy from an American Physical Therapy Association (APTA)-approved program - Required

Key Responsibilities

• Provide physical therapy services to patients in their homes.

• Evaluate patients and develop individualized treatment plans based on patient needs and physician orders.

• Deliver therapeutic interventions to improve mobility, strength, balance, and functional independence.

• Monitor patient progress and modify treatment plans as appropriate.

• Complete accurate documentation of evaluations, treatments, progress notes, and outcomes.

• Collaborate with physicians, nurses, caregivers, and other members of the healthcare team.

• Educate patients and caregivers on exercises, mobility techniques, safety precautions, and home exercise programs.

• Travel throughout assigned service areas to provide patient care.
