Position Overview

ProTouch Staffing is seeking an experienced Registered Nurse Case Manager (RN CM) for a travel assignment at University of Virginia Medical Center in Charlottesville, VA.

The RN Case Manager will provide comprehensive inpatient case management services, coordinate patient care across the healthcare continuum, and work closely with physicians, multidisciplinary teams, patients, and families to support safe and appropriate discharge planning.

Requirements

• Active RN license

• Acute care hospital inpatient Case Management experience - REQUIRED

• Strong experience with inpatient case management and discharge planning

• Must have either travel nursing experience OR Epic experience

• Ability to collaborate effectively with physicians and multidisciplinary healthcare teams

• Strong communication, organization, and clinical assessment skills

• Must be available to work 2 out of the 3 winter holidays

Key Responsibilities

• Perform comprehensive case management assessments for hospitalized patients.

• Coordinate patient care and discharge planning throughout the inpatient stay.

• Collaborate with physicians, nurses, social workers, and other healthcare professionals

• Identify barriers to discharge and develop appropriate plans to address them.

• Coordinate post-acute services, referrals, and appropriate levels of care.

• Monitor patient progress and ensure appropriate utilization of healthcare resources

• Communicate effectively with patients and families regarding care plans and transitions.

• Maintain accurate and timely documentation in the electronic medical record.

• Support quality, safety, and continuity of care throughout the patient journey
