Schedule:

• Monday - Friday

• 8:00 AM - 5:00 PM

• 40 hours per week

Pay:

• Pay Rate + IRS Mileage Reimbursement: $0.76/mile

Job Details:

• Guaranteed Work Week: 40 hours.

• Coverage Area: Marin County.

• May be required to assist in additional territories within the facility's service area as needed, approximately within a 60-mile radius from the facility address.

• Average daily caseload: 4-6 visits per day depending on the type of visit for PT/OT/SLP.

• Productivity expectation: 28-32 points per week for PT/OT/SLP.

Productivity Breakdown:

• Start of Care (SOC): 2.5 points

• Resumption of Care (ROC): 2 points

• Evaluations/Recertification/Discharge OASIS: 1.5 points

• Discipline Discharge, Routine Visit, 30-Day Reassessment: 1 point

• Weekly Case Conference: 1 point

Requirements:

• Home Health experience required.

• OASIS experience required.

• No exceptions.
