RN Clinical Manager - Home Health

Location: Lake Oswego, OR
Position Type: 13-Week Contract | Local or Travel
Schedule: Monday-Friday, 8:00 AM-5:00 PM
Hours: 40 hours per week
Setting: In-Office / Home Health
Potential: Contract-to-Permanent opportunity for the right candidate

Position Overview

ProTouch Staffing is seeking an experienced RN Clinical Manager for a Home Health opportunity in Lake Oswego, Oregon.

The ideal candidate will have strong experience in Home Health clinical operations, Homecare Homebase (HCHB), OASIS, case management, and clinical staff supervision.

The Clinical Manager will oversee clinical processes, ensure appropriate patient care and documentation, coordinate referrals and case conferences, review orders, and support compliance with applicable home health regulations.

Key Responsibilities

Clinical

• Ensure each client's plan of care is individualized, appropriate, implemented as written, and reassessed when required.

• Review evaluation packets, OASIS, and 485s.

• Review documentation submitted through mobile devices and paper records for accuracy.

• Process OASIS and 485s and verify correct start-of-care and episode dates in HCHB.

• Follow up on documentation deficiencies that prevent OASIS processing.

• Manage referral intake and ensure timely scheduling and completion of assessments.

• Lead weekly case conferences and address recertification and discharge decisions.

• Review and approve case conference coordination notes.

• Contact physicians to obtain orders for continued or additional services.

• Review, approve, and track physician orders in HCHB.

• Maintain accurate medication profiles and schedules.

• Ensure frequency orders have appropriate calendar modifications.

• Review vital-sign alerts and wound-score deviations and document appropriate follow-up.

• Participate in patient visits and/or on-call rotation as needed.

Administrative

• Review entitlement verification issues and document appropriate follow-up.

• Verify benefits and obtain authorization for non-Medicare services.

• Review on-call coordination notes and follow up on identified deficiencies.

• Address billing claim audit deficiencies within required timeframes.

• Run missed-visit reports and submit them to physicians.

• Provide backup support for the Patient Services Coordinator.

• Review lab reports and coordinate physician follow-up when required.

• Function as a staff nurse PRN as needed.

Required Qualifications

• Active RN license

• RN license in the state of employment

• Minimum 2 years of nursing experience

• Minimum 1 year of Home Health experience

• Home Health experience - Required

• OASIS experience - Required

• Homecare Homebase (HCHB) experience - Required

• Home Health case management experience

• Clinical staff supervision experience preferred

• Knowledge of federal, state, and local regulations governing Home Health agencies

• Strong organizational and communication skills

• Excellent documentation and computer skills

• Ability to multitask in a professional environment

• Strong interpersonal and customer service skills

Additional Requirements

• Valid state driver's license

• Valid automobile liability insurance

• Ability to drive an automobile in various weather conditions

• Willingness to work occasional evening/weekend on-call as needed
