Klarus Home Health is seeking a PRN / Per Diem Licensed Occupational Therapist to join our team to providing OT services including assessment, treatment planning, implementation, and evaluation for our patients currently enrolled in our home health program in the Cleburne (Johnson County), TX and surrounding areas.

Requirements
Key Responsibilities:
- Conduct ongoing clinical assessments, including OASIS assessments, to identify patient/client needs and determine levels of functioning.
- Evaluate patients’ occupational performance in areas such as self-care, work, leisure, and rest.
- Assess sensory-motor neuromuscular, psycho-social, and cognitive-perceptual components of occupational behavior.
- Evaluate physical, social, and cultural components of patients’ occupational environment.
- Facilitate direct therapeutic interventions to address ROM, strength, coordination, adaptive equipment needs, and environmental modifications.
- Develop, evaluate, and revise the plan of care in collaboration with patients, representatives, and caregivers.
- Document clinical notes, phone orders, and other records in a timely manner.
- Accept clinical assignments consistent with education and competence.
- Provide physician-ordered services, including patient/client, caregiver, and family counseling and education.
- Supervise and guide occupational therapy assistants and home health aides as appropriate.
- Apply infection control and standard precautions to prevent contamination and transmission of disease.
- Utilize effective interpersonal relations and communication skills to achieve desirable outcomes.
Job Qualifications:
- Education: Bachelor of Science in Occupational Therapy.
- Licensure: Current State License as an Occupational Therapist, Current driver’s license required.
- Experience: One year of experience as an Occupational Therapist preferred. Home health experience preferred
- EMR: Home care home base proficiency preferred
- Skills: Proficiency in therapy skills as per accepted standards of practice, good interpersonal skills, current CPR certification, and knowledge of durable medical equipment.
- Transportation: Reliable transportation and valid/current auto liability insurance.
