Description
Responsibilities:
- Assess patients' needs and develop personalized treatment plans in the home setting
- Provide therapy to improve daily living skills and overall functions
- Collaborate with caregivers and families to support patient goals
- Educate patients and families on adaptive strategies and home modifications
- Monitor and document patient progress and outcomes regularly
- Implement interventions tailored to the home environmenty
- Participate in care coordination with healthcare teams and community resources
- Master's degree in Occupational Therapy
- WA Occupational Therapy Licensure
- Clean driving record
- Pay: $1,433.34 - $1,477.67 / week
- Location: Vancouver, WA
