Description

Responsibilities:
  • Assess patients' needs and develop personalized treatment plans in the home setting
  • Provide therapy to improve daily living skills and overall functions
  • Collaborate with caregivers and families to support patient goals
  • Educate patients and families on adaptive strategies and home modifications
  • Monitor and document patient progress and outcomes regularly
  • Implement interventions tailored to the home environmenty
  • Participate in care coordination with healthcare teams and community resources
Qualifications:
  • Master's degree in Occupational Therapy
  • WA Occupational Therapy Licensure
  • Clean driving record
