Description

If you're interested in making a significant impact as a Physical Therapist (PT) in Home Health setting this Portland, OR position might be what you are looking for!

Responsibilities:

Assess patients' physical abilities and develop personalized treatment plans in the home setting
Provide therapy to improve mobility, strength, and functional independence
Collaborate with patients, families, and caregivers to support rehabilitation goals
Educate patients and families on exercise programs and safety measures
Monitor and document patient progress and adjust treatment plans as needed
Implement interventions tailored to the home environment and lifestyle
Coordinate care with healthcare teams and community resources for comprehensive support

Qualifications:

Doctorate in Physical Therapy
OR Physical Therapist PT license
Current BLS/CPR Certification
Clean Driving Record
