Description

Responsibilities:

Assess patients' needs and develop personalized treatment plans in the home setting
Provide therapy to improve daily living skills and overall functions
Collaborate with caregivers and families to support patient goals
Educate patients and families on adaptive strategies and home modifications
Monitor and document patient progress and outcomes regularly
Implement interventions tailored to the home environmenty
Participate in care coordination with healthcare teams and community resources

Qualifications:

Master's degree in Occupational Therapy
PA Occupational Therapy Licensure
Clean driving record
