Responsibilities:
- Assess patients' physical abilities and develop personalized treatment plans in the home setting
- Provide therapy to improve mobility, strength, and functional independence
- Collaborate with patients, families, and caregivers to support rehabilitation goals
- Educate patients and families on exercise programs and safety measures
- Monitor and document patient progress and adjust treatment plans as needed
- Implement interventions tailored to the home environment and lifestyle
- Coordinate care with healthcare teams and community resources for comprehensive support
- Doctorate in Physical Therapy
- VT Physical Therapist PT license
- Current BLS/CPR Certification
- Clean Driving Record
- Pay: $2,386.85 - $2,460.67 / week
