Opportunity:

Physical Therapist - Home Health

Job Details:

• Monday through Friday schedule, 8:00 AM to 5:00 PM with no weekend or holiday shifts required
• Home health care setting with majority of work focused on home health visits
• Expected to complete 6.5 points per day
• Regular daily visits consist of 5 to 6 patients per day when no additional travel or assessments are involved
• When admissions are scheduled, 3 additional visits are expected
• Responsible for start of care, recertifications, and resumption of care visits
• May occasionally be asked to complete a safety evaluation or teaching visit in a hospice situation
• Home health and OASIS training experience preferred
• Resources available while in the field including physical therapists from sister agencies, a Clinical Coordinator, and a Director
• Orientation period of 1 to 2 weeks working alongside a preceptor
• Scrub colors required: black, navy blue, or grey
• Must have a current driver's license and valid auto insurance
• Required certifications: BLS, ND Physical Therapy License
• Charting completed using a computerized system
