Opportunity:

PT - Home Health

Job Details:

• Work Monday through Friday, 8:00 AM to 5:00 PM
• No weekend or holiday requirements
• Treat 5-7 patients per day in a home health setting
• Cover a 60-mile radius around Valley City, ND
• Common patient diagnoses include post-surgical recovery and fall-related injuries
• Serve home health and hospice patients
• Must have prior home health experience
• Minimum 1 year of physical therapy experience required
• Prior travel experience not required
• Charting system: HomeCare/HomeBase
• 3-5 shifts of department-specific orientation provided
• No physical therapy manager or supervisor on shift
• Dress code: CSH color scrubs or business casual
• Must have a current driver's license and valid auto insurance
• Required certifications: BLS, ND Physical Therapy License
