Opportunity:

Home Health PT - Medford OR and Surrounding Areas

Job Details:

• Work 8-hour day shifts with immediate start date available
• Visit up to 6 patients per day with 30 productivity points standard
• 1:1 patient ratio during home visits
• Guaranteed hours with facility able to call off up to one shift per pay period (every two weeks)
• 1 year of skilled home health experience required
• First-time home health travelers not accepted
• BCLS certification required
• Oasis experience required
• Must have reliable personal transportation as occasional visits may require travel up to 50+ mile radius
• Any color scrubs acceptable
• Charting system: EPIC/Oasis
• Free parking available
• IRS standard mileage reimbursement provided
• Weekend coverage not required
• No on-call responsibilities
• No floating between units required
• Block scheduling not accommodated
• 4x10 or 3x12 schedules cannot be accommodated
• Must complete non-billable modules averaging 16 hours pre-start and annually
