Opportunity:

Physical Therapist - Home Health

Job Details:

• Provide physical therapy services across an assigned county territory covering multiple areas
• Primary coverage area includes Richmond, Rodeo, El Cerrito, El Sobrante, Hercules, Pinole, San Pablo, Crockett, and Kensington
• Secondary coverage in either Oakland, Berkeley, Alameda, Albany, Emeryville, and Piedmont or Vallejo, Benicia, Napa, American Canyon, and Yountville
• Additional travel outside assigned area may be required
• Weekend coverage may be required
• Guarantee option requires a minimum of 27 points per week with a 36-hour weekly guarantee
• Must be willing to travel outside the service area or work weekends if required to maintain weekly visit expectations under the guarantee option
• Non-guarantee option allows patient refusal with no guaranteed hours and payment made on a per-visit basis
• Visit types include OASIS ReCertification, OASIS RoC, Routine, Eval, Discharge, Eval SoC, Eval RoC, Eval ReCert, Eval F/up, Re-Eval, Eval Discharge, OASIS Discharge, and OASIS F/up
• Hours are calculated based on visit type: 1 point equals 1.33 hours and OASIS SoC at 1.5 points equals 2 hours
• Rate is all-inclusive and covers drive and travel time, visit time, documentation time, and time spent making phone calls to patients and physicians
• Required credentials: Current California PT License, BLS certification, valid Driver's License, auto insurance, auto registration
