Opportunity:

Home Health PT - West Portland OR and Surrounding Areas

Job Details:

• Work 8-hour day shifts (facility cannot accommodate 4x10s or 3x12s)
• Visit up to 6 patients per day with 30 productivity points standard
• 1:1 patient ratios with individual home visits
• No weekend coverage required
• No on-call responsibilities
• No floating between units required
• Guaranteed hours with facility able to call off up to one shift per pay period (every two weeks)
• Must have own reliable transportation for patient visits
• Maximum travel radius of roughly 60 miles (60-90 minute total drive time) per day
• Business casual attire required, no jeans, scrubs allowed but not required
• Free parking provided
• Mileage reimbursement at IRS standard rate
• Oregon license required
• 1 year of skilled home health experience required
• BCLS certification required
• OASIS documentation experience required
• Epic Remote Client for Home Health charting on laptop and Rover phone app
• Experience with lifting and transfers, ordering supplies, and scheduling appointments required
• Wound care and manual therapy certifications preferred
• Must complete non-billable modules averaging 16 hours pre-start and annually
