PT- Home Health- Day Shift

Job Details:

• Part-time position working Monday through Friday from 8:00 AM to 5:00 PM
• HomeCare HomeBase experience required
• EPIC experience required
• Minimum 2 years experience in the submitted unit specialty within the last 3 months required
• BLS certification required
• Valid driver's license required with minimum age of 21 years
• Current automobile insurance and reliable automobile required
• Must provide RTO, birthdate and social security number
