Opportunity:
Home Health PT - Seaside OR and Surrounding Areas
Job Details:
• Work 8-hour day shifts (facility cannot accommodate 4x10s or 3x12s)
• Visit up to 6 patients per day with 1:1 patient ratios
• 30 productivity points standard required
• Minimum 1 year of skilled home health experience required
• First-time travelers accepted
• No weekend requirements
• No on-call responsibilities
• No floating between units required
• BCLS certification required
• Oasis documentation required
• Must have reliable personal transportation
• Maximum driving radius of approximately 60 miles per day (60-90 minute total drive time)
• Business casual attire required, no jeans allowed, scrubs permitted but not required
• Use EPIC Remote Client for Home Health on laptop and Rover phone app for documentation
• Free parking provided
• IRS standard mileage reimbursement
• Facility may call off up to one shift per pay period (every two weeks)
• Must complete non-billable modules averaging 16 hours pre-start and annually
• Two professional references required covering 1 year within the last 3 years
