Opportunity:

Physical Therapist - Home Health

Job Details:

• Work 8:30 AM to 5:00 PM shifts, typically Monday through Friday
• Saturday rotation approximately once per month
• Provide physical therapy services in home health setting throughout Yolo County service area
• Travel to patient homes in territories including Woodland, Davis, Yolo, Dunnigan, Arbuckle, West Sacramento, Natomas, Knights Landing, Capay, Vacaville, Winters, and Esparto
• Minimum 2 years experience as PT in acute care, home care, or rehabilitation setting required
• BLS certification required
• Must provide 2 supervisor references from PT modality: 1 within 12 months and 1 within 3 years
• Assess and evaluate patients' therapeutic, rehabilitative, and functional status
• Develop treatment plans according to physician's plan of care
• Evaluate home environments and assess muscle strength, mobility, gait, and range of motion
• Instruct patients and families on use and care of therapeutic appliances
• Document patient progress and report changes to attending physicians
• Supervise home health aides when participating in patient rehabilitation
• Use Homecare Homebase (HCHB) charting system
• Dress code: scrubs optional (any color), or slacks with polo/button-up shirt
• Mileage reimbursement available per GSA guidelines starting after first patient visit
• Good oral and written communication skills required
