Opportunity:
Home Health OT
Job Details:
• 8-hour day shifts (facility cannot accommodate 4x10s or 3x12s)
• 1:1 patient ratio with up to 6 patients visited per day; 30 productivity points standard
• No weekend requirements
• No on-call requirements
• Facility may call off up to one shift per pay period (every two weeks)
• Traveler must have their own reliable transportation
• Occasional maximum travel radius of roughly 60 miles (60-90 minute total drive time) per day; system groups clients to minimize drive and windshield time
• Both Oregon and Washington licensure required (WA license in hand preferred; OR license can be pending)
• Minimum 1 year of skilled home health experience required
• OASIS experience required
• Required certifications: BLS
• Charting systems: EPIC Remote Client for Home Health (on laptop) and Rover (phone app)/OASIS
• Attire: Business casual; no jeans; scrubs allowed but not required (no specific color)
• Mileage reimbursement provided at IRS standard rate
• Free parking
• Patient diagnoses include: Dementia, Parkinson's, ALS, MS, ortho post-op, and post-stroke
• Patient population: Adults and geriatrics
• Preferred skills include: National Patient Safety Goals/Core Measures, billing, home health, and OASIS documentation
• Preferred activities of daily living experience: Acute disorder, chronic disorder, and functional activities
• First-time travelers accepted
• Local candidates accepted (considered local within 50-75 mile radius at the same rate)

This home health facility provides patient-centered care delivered directly in patients' homes across Hood River, OR and the surrounding areas. The organization serves a diverse patient population with complex diagnoses and utilizes a structured productivity model to ensure efficient and quality care delivery. The facility leverages modern technology including a remote EPIC charting system and mobile applications to support clinicians in the field.
