Opportunity:

Home Health PT - East Portland OR and Surrounding Areas

Job Details:

• 8-hour day shifts (facility cannot accommodate 4x10s or 3x12s)
• May visit up to 6 patients per day with 30 productivity points as standard
• Patient population includes adults and geriatrics with diagnoses such as Dementia, Parkinson's, ALS, MS, orthopedic post-op, and post-stroke
• No weekend requirements
• No on-call requirements
• Facility may call off up to one shift per pay period (every two weeks)
• Traveler is expected to have their own personal transportation
• Occasional maximum radius of roughly 60 miles (60-90 minute total drive time) per day; system groups clients to minimize drive and windshield time
• OASIS experience required
• Charting completed via EPIC Remote Client for Home Health (on laptop) and Rover phone app/OASIS
• Business casual attire required; no jeans; scrubs allowed but not required with no specific color requirement
• Mileage reimbursement provided at IRS standard rate
• Free parking available
• 1 year of skilled home health experience required
• Epic charting system experience preferred
• Required certifications: BLS
