Job Details:

• Day shift schedule
• Mileage reimbursement included in addition to base rate
• Physical Therapy licensure in all applicable states of practice required
• Doctor of Physical Therapy (DPT) degree required if license obtained on or after January 2016, or foreign equivalent
• Cardiopulmonary Resuscitation (CPR) certification or Basic Life Support (BLS, HS-BLS, or RQI BLS) certification from approved vendor required
• Valid Driver's License required
• Must be at least 21 years of age or older
