Opportunity:

Physical Therapist - Home Health

Job Details:

• Monday through Friday, 8:00 AM to 5:00 PM with rotating weekend shifts (8:00 AM to 5:00 PM)
• 5 eight-hour days per week
• Must complete 6.5 points per day including Start of Care (SOC), Resumption of Care (ROC), recertifications, evaluations, and daily visits
• Holiday rotation required
• 2-week orientation with a work coach, including a few supervised visits
• Home Health care setting covering patients with diagnoses including CHF, orthopedic conditions, and pneumonia
• Charting system: Homecare Homebase (HCHB) — prior experience with this system is required
• Home Health experience required
• No new graduates preferred
• Clinical manager or educator available 24/7 for emergencies and 8:00 AM to 5:00 PM for general support
• Phone and tablet will be provided
• Either scrubs or business casual attire is acceptable
• Required certifications: BLS
• Active license in hand required upon submission
• BSN not required
