Physical Therapy

Job Details:

• Work 8-hour shifts Monday through Friday from 8:00 AM to 5:00 PM with rotating weekends
• Provide 6.5 visits per day including start of care, recertification, evaluations, and daily visits
• Cover home health area with 24/7 clinical manager or educator support available for emergencies (8-5 PM for routine support)
• Two weeks of orientation with workcoach supervision including few visits with preceptor
• BLS certification required
• No new graduates preferred but not strictly required
• Treat patients with common diagnoses including CHF, orthopedic conditions, and pneumonia
• Participate in holiday rotation schedule
• Use Homecare Homebase (HCHB) charting system
• Phone and tablet will be provided
• Dress code allows either scrubs (any color) or business casual
• BSN not required
• Valid license required for submission
