Opportunity:

Home Health OT - East Portland OR and Surrounding Areas

Job Details:

• 8-hour day shifts (4x10 and 3x12 schedules cannot be accommodated)
• May visit up to 6 patients per day with 30 productivity points as standard
• 1 year of skilled home health experience required
• No weekend requirements
• No on-call requirements
• Guaranteed hours: Facility may call off up to one shift per pay period (every two weeks)
• Traveler must have their own personal transportation
• Occasional maximum radius of roughly 60 miles (60-90 minute total drive time) per day; system groups clients to minimize drive and windshield time
• Charting system: EPIC Remote Client for Home Health (on laptop) and Rover phone app/OASIS
• OASIS experience required
• Attire: Business casual; no jeans; scrubs allowed but not required and no specific color required
• Free parking available
• Mileage reimbursement at IRS Standard rate
• Required certification: BLS
• Patient population includes adults and geriatrics
• Patient diagnoses include dementia, Parkinson's, ALS, MS, ortho post-op, and post-stroke
• Required skills: Infection prevention and fall/risk assessment
• Preferred skills: National Patient Safety Goals/Core Measures, billing, home health, and OASIS documentation
