Opportunity:

PT - Home Health

Job Details:

• Work Monday through Friday, 8:30 AM to 5:00 PM with no weekend or holiday requirements
• See 4-6 patients per day with the ability to start shifts from home without stopping at an office first
• Travel within a 50-mile radius east and west, approximately 60 miles north up to Sun Valley, and approximately 40 miles south to the ID/NV border (no mileage reimbursement provided)
• Treat a multidisciplinary adult patient population consisting of approximately 78% Medicare patients including elderly, rehabilitation, CVA, and inpatient surgical referrals covering orthopedic, surgical, and wound cases (no lymphedema)
• Perform admissions, treat existing patients, and complete recertifications including a limited number of assisted living patients
• Handle limited oxygen requirement cases
• Participate in multidisciplinary meetings every Monday and Wednesday to discuss current admissions
• Document using Epic with remote client offshoot
• Complete department-specific orientation including Epic classes and up to one week shadowing with another therapist
• Supervisors available for support throughout assignment
• Must have a minimum of 1 year post-graduate physical therapy experience
• Must have home health experience, with post-graduate home health experience preferred
• Must have orthopedic experience

• Required certifications: BLS (AHA)
• Cancellation policy: 4 shifts per 13-week assignment
