Job Details:

• Travel experience required
• Keep requested time off to a minimum, preferably no requests for a full calendar week off during assignment
• Any requested time off must be included in the initial submission and approved in advance of offer

• HomeCare HomeBase charting system experience preferred
• 2 supervisory references required
• Graduate degree from an accredited Physical Therapy Program approved by the APTA required
• Minimum of one year of physical therapy experience, home health or facility preferred
• Current and unrestricted Physical Therapy Licensure required (temp or pending licenses accepted, or confirmation that license will be in hand prior to start)
• Current CPR certification required
• ARC certifications accepted
