Opportunity:

RN - Nurse Navigator (Telehealth)

Job Details:

• Work 8-hour day shifts from 9:00 AM to 5:30 PM
• Schedule consists of 5x8-hour shifts per week
• Non-patient-facing telehealth position
• Serve as a dedicated clinical liaison for patients and their families
• Guide patients through complex diagnoses, treatment plans, and follow-up care
• Patient population includes adults and geriatrics
• Up to 11 patient visits per day
• Utilize good nursing judgment within the framework of Clinical Evaluation/Competency Center (CECC)
• Must be proficient in navigating computer systems; Omnicell medication system utilized
• Minimum 1 year of Med-Surg experience required
• Experience in Cardiology, Family Care, or Internal Medicine settings preferred
• Previous charge experience preferred
• Community hospital experience preferred
• Bilingual in Spanish highly preferred
• Required certifications: BLS and ACLS (AHA)
• Workers should expect to work some holidays during the assignment
