Physical Therapist - Home Health

Job Details:

• Full-time position working day shifts
• Average patient visit is 45 minutes allowing for meaningful therapy sessions
• Evaluate patients' need for physical therapy and develop appropriate treatment plans
• Carry out treatment programs in accordance with agency policies and professional standards of care
• Communicate and coordinate treatment activities with interdisciplinary team members and patients
• Document evaluations, treatment goals, and objectives
• Teach patients and family members the rationale for treatments and procedures
• Screen, evaluate, re-evaluate, and treat patients across multiple service locations
• Work with broad independence while remaining part of a cohesive patient-care team
• Bachelor of Science in Physical Therapy required
• Must hold an active Physical Therapist license in Vermont and New Hampshire, or an unencumbered compact license
• Minimum of one year of experience as a Physical Therapist in a home healthcare setting required
