**Exciting Opportunity for a Case Manager in California!**

Job Description:

- **Shift:** Day 5x8-Hour (08:00 - 17:00)
- **Experience:** Minimum of 3 years in acute care case management
- **License:** Must possess an active California RN License
- **Certifications:** BLS required
- **Flu Vaccine:** Mandatory (no exemptions)
- **Special Requirements:**
- EPIC experience highly preferred
- California case manager experience is essential

**Job Details:**

- **Weekend Requirement:** No
- **Floating Requirement:** No
- **On-Call Requirement:** No
- **Pending License:** Not accepted; must be active at time of submission
- **Flexible Scheduling:** No scheduling requests can be accommodated

**Skills & Qualifications:**

- Previous charge experience is a plus
- Familiarity with patient ratios
- Experience with charting systems (Epic preferred)
- Knowledge of case management best practices, including:
- Care coordination
- Discharge planning
- HIPAA guidelines
- CMS, CPT, DRG, and ICD 10 coding
- Utilize InterQual Criteria and Milliman Guidelines
- Workers Compensation

**Unit Details:**

- **Patient Age Groups:** Adolescents, Adults, Geriatrics
- **Daily Census:** 255
- **Number of Visits Per Day:** 15
- **Number of Beds:** 14

**Additional Information:**

- **Interdisciplinary Support:** Includes social services and transportation
- **References:** Two references required from within the past year (must be a supervisor or higher)
