• Job Title: Hospice Registered Nurse (RN) – Travel/Contract
• Location: Girard, OH 44420 (On-site)
• Territory: Trumbull, Mahoning, and Columbiana Counties
• Assignment Length: 13 weeks
• Start Date: November 3, 2025
• Shifts per Week: 4 (Friday, Saturday, Sunday, Monday)
• Shift Duration: 10-hour days (10:00am – 8:00pm)
• Hours per Week: 40
• Estimated Weekly Pay: $1,692 – $1,880
• Travel and mileage reimbursement for healthcare professionals
• Overtime & Holiday Rate: 1.3x standard rate
• Callback Rate: 1.0x standard rate

Registered Nurse Qualifications

• Minimum 1 year of recent hospice RN experience (within the past 3 years)
• Proficiency with HCHB (Homecare Homebase) – required
• Experience with OASIS documentation – required
• Active Ohio or Compact Registered Nurse License (must be in hand at time of application)
• Current BLS certification

Hospice RN Responsibilities

• Provide compassionate hospice nursing care in home and facility settings throughout Northeast Ohio
• Conduct comprehensive patient assessments and create individualized care plans
• Document patient care accurately using HCHB and OASIS systems
• Collaborate with interdisciplinary healthcare teams for high-quality, patient-centered care
• Educate patients and families about hospice services, symptom management, and end-of-life care
• Ensure compliance with regulatory and organizational standards
