Registered Nurse Qualifications

• Active RN license in the state of New Jersey
• Minimum 2 years of recent experience as a Home Health Registered Nurse
• Proficiency with OASIS and HCHB (Homecare Homebase) documentation systems
• Current CPR certification
• Valid driver’s license, vehicle insurance, and reliable transportation

Home Health RN Responsibilities

• Deliver skilled nursing care to patients in their homes, following individualized care plans
• Perform comprehensive patient assessments and document findings using OASIS and HCHB
• Collaborate with interdisciplinary healthcare team members to ensure optimal patient outcomes
• Educate patients and families on disease management, medications, and self-care techniques
• Monitor patient progress, update care plans, and communicate changes to the healthcare team
• Maintain accurate, timely clinical documentation in compliance with home health regulations
