Social Worker Qualifications
- Master's of Social Work (MSW) or Bachelor's degree in a related field with clinical experience
- Active LMSW license (Licensed Master Social Worker)
- Minimum 1 year of experience in social work, case management, utilization review, or related healthcare field (internship experience accepted)
- Strong communication skills for interacting with patients, families, physicians, and hospital staff
- Certification as a Certified Case Manager (CCM) preferred
- Three years of experience in case management or social services preferred
Social Worker Responsibilities
- Coordinate care for neonate, adolescent, adult, and geriatric patients; assess post-hospital needs
- Provide crisis intervention, psychosocial assessments, and adjustment support during hospitalization
- Screen cases for preadmission, admission, concurrent, and post-discharge per healthcare policies
- Collaborate with staff to identify discharge planning priorities and investigate post-hospital care options
- Conduct interviews with patients and families to assess physical/mental health, family dynamics, support systems, and financial status
- Issue Notice of Non-coverage letters and discuss financial responsibility as needed
- Coordinate with healthcare and community agencies to implement discharge plans
- Document assessments, progress, and discharge plans in patient charts; communicate updates to all parties
- Refer to Risk Management, Infection Control, Pharmacy, and other clinical support areas as needed
- Assist families with financial difficulties and connect them to appropriate resources
- Report suspected abuse or neglect according to hospital policy
- Maintain updated resource files and ongoing communication with referral agencies
- Respond to internal and external referrals, providing resource information
- Participate in studies to evaluate social work services and develop support materials
- Provide in-service education for hospital staff on case management and clinical resource management
- Assist in appealing denials attributed to Care Coordinator
- Practice safety techniques and report unsafe conditions promptly
- Perform other duties as assigned
