Social Worker Job Details

• Setting: Home Health & Hospice (inpatient and community-based)
• Assignment Length: 12-week contract
• Start Date: January 12, 2026
• Schedule: 5 shifts per week, 8-hour days (Monday to Sunday rotation)
• Hours per Week: 40
• Estimated Weekly Pay: $2,447 - $2,718
• On-Site Position: Reliable transportation required; winter driving conditions expected

LMSW Qualifications

• Master’s Degree in Social Work (MSW)
• Active Vermont LMSW license
• Minimum 2 years of clinical experience in home health, hospice, or related healthcare settings
• BLS certification
• Strong assessment, counseling, and critical thinking skills
• Experience with end-of-life care planning and goals-of-care discussions
• Ability to drive in winter conditions; reliable vehicle with snow tires
• Complete work history and professional references required

Social Worker Responsibilities

• Deliver psychosocial support, counseling, and resource coordination for patients and families facing terminal illness
• Conduct comprehensive assessments and create individualized care plans
• Collaborate with interdisciplinary healthcare teams to manage complex symptoms and ensure quality hospice care
• Educate patients and caregivers on hospice philosophy, symptom management, and available resources
• Document care and interventions using Net Smart EMR system
• Cross-cover adult home health, inpatient hospice (McClure Miller Respite House), and community hospice as needed
• Participate in weekend and holiday rotations
• Maintain professional appearance and adhere to dress code (business casual or non-institutional scrubs)
