LCSW Qualifications

• Active LCSW (Licensed Clinical Social Worker) license – required for Ohio social worker jobs
• Minimum 1 year of experience as a social worker in a healthcare setting
• Experience in discharge planning and case management preferred
• Proficiency with EMR systems (Med Host experience a plus)
• Excellent communication and organizational skills
• Ability to manage a caseload of 10-15 patients per day

Social Worker Responsibilities

• Deliver comprehensive discharge planning and case management services to patients
• Collaborate with a multidisciplinary healthcare team, including two RN Case Managers
• Assess patient needs and develop individualized care plans
• Coordinate with healthcare providers, patients, and families for seamless care transitions
• Document all interventions and communications in the EMR (Med Host)
• Participate in orientation during the first week and receive ongoing clinical support
