• Shifts per Week: 5
• Shift Duration: 8 Hour Days
• Scheduling: Weekdays (5 days/week) or 4x10hr shifts (Thur-Sun or Fri-Mon)
• Estimated Weekly Pay: $1,799 - $1,999

LMSW Job Requirements

• Active Licensed Master Social Worker (LMSW) license in Virginia or compact state (required at submission)
• Minimum 2 years of clinical experience in an acute hospital setting
• BLS and ACLS certifications (required)
• Proficiency with Meditech EMR and CarePort Care Management documentation systems
• Knowledge of AIDET patient experience standards
• ACMA (American Case Management Association) or CCM (Certified Case Manager) certifications preferred

Social Worker Responsibilities

• Conduct initial patient evaluations and reassessments for discharge planning
• Coordinate referrals for post-acute healthcare resources and placements, including home health, DME, home infusion, hospice, skilled nursing facility (SNF), and long-term care
• Arrange discharge transportation and facilitate smooth transitions of care
• Document all hospital case management activities using Meditech and CarePort Care Management systems
• Float within the Case Management Department, supporting ER, NICU, ICU, PCU, and Med-Surg units
• Collaborate with Case Management Assistants, Managers, and Directors to address patient needs
