Personal Health History

• COVID-19 Vaccine: Proof of September 2025 booster, declination or exemption with agency letterhead (declination not accepted for CA or IL).

• Fit Test: Required for Iowa (1860 mask only).

• Influenza Vaccine: Proof of vaccine, declination, or exemption with agency letterhead (declination not accepted for CA or IL). Valid for flu season 10/1/2024 - 4/30/2025.
• 2 Step PPD OR IGRA (Quantiferon/Tspot): Required. Positive TB requires chest x-ray within last year for Michigan and certain states; otherwise within 5 years.
• Varicella: Proof of 1 vaccination with date or IgG titer lab report.
• Physical/Health Statement: Documentation from exam within last year confirming fitness to work.
• Drug Screen: 9 or 10 panel drug screen completed within 30 days of start (preferred within 14 days), must include specific substances; no THC allowed.
• Tdap: Current, non-expired vaccine required. Declinations accepted if no pediatric interaction.

Certifications & Licenses

• BLS: American Heart Association certification required for all direct patient care personnel. Must provide front and back images of card.
• License/Certificate Verification: Must be dated within 30 days of start. Attach scanned online verification with machine-generated date. For California, a picture of the hard copy license is also required.

Professional Requirements

• Resume and References: Resume, skills checklist within a year, and 2 professional references required.
