Social Worker Job Details – Chicago, IL

Location: Chicago, IL 60637 – Explore Chicago healthcare jobs in Hyde Park, close to Lake Michigan, cultural attractions, and public transportation.
Employment Type: Travel Contract
Schedule: Monday - Friday
Shift: 8:00am - 4:30pm (8 Hour Days)
Shifts per Week: 5
Hours per Week: 40
Estimated Weekly Pay: $1,930 - $2,144
Assignment Duration: Minimum 13 weeks, with potential extension

Social Worker Qualifications

Active Illinois Social Work License (LCSW or LSW required)
Minimum 1 year of clinical social work experience in a healthcare setting
Experience with ventricular assist device (VAD) care preferred
Experience supporting heart failure patients preferred
Strong communication, organizational, and case management skills

Social Worker Responsibilities

Deliver social work services to heart failure and VAD patients and their families
Perform psychosocial assessments and develop individualized care plans
Collaborate with interdisciplinary healthcare teams for patient care and discharge planning
Provide emotional support, counseling, and resource referrals
