Candidate Requirements

Licensure & Certifications

• Active Arizona RN license required at time of submission (verification must be less than 30 days old)
• Compact license allowed
• No license hits allowed
• Certifications required (must be active at time of submission):

  • ACLS (American Heart Association)
  • BLS (American Heart Association)
• Certifications must be from American Heart Association or American Red Cross and included in submission

Experience

• Minimum 24 months of current Progressive Care Unit (PCU) experience required
• Travel experience preferred

Compliance & Documentation

• Candidates with an Arizona home address on their driver’s license will not be considered (copy of driver’s license required in submission)
• Banner has very specific background requirements that must be adhered to and attested
• Flu vaccination required; declinations can be applied for (vaccination status must be noted in submission)
• Return to Office (RTO) requests:

  • Not approved within the first week of assignment or during holidays
  • Cannot exceed 7 days
• Learning modules must be completed prior to assignment start

Submission Requirements

• Fully completed Banner cover page including:

  • All locations and departments candidate is willing to work in
  • All shifts candidate is open to working
  • All requested information noted in highlights section
• Resume must include:

  • All facilities worked at
  • Facility size, bed count, trauma level, and units listed
• Skills checklist specific to nursing discipline (must be less than 1 year old)
• Active driver’s license copy included

Additional Notes

• Start date should be on or very soon after the order start date
• Non-billable hours adjusted to 16 hours per assignment
• Assignments may be canceled with 7 days written notice without penalty
• Banner assesses a 2% payment discount fee and a 2% technology fee on all transactions
