Job Overview

• Position is for an external traveler to cover a resignation.
• Currently experiencing 48% staffing shortage on the night shift.

Required Credentials and Certifications (Must be current and verified within specified timeframes)

Certifications

• BLS (American Heart Association certification required for all direct patient care personnel; front and back of card needed)
• ACLS
• NRP (Neonatal Resuscitation Program)
• AWHONN/Fetal Heart Monitoring

Licenses

• Valid nursing license with verification dated within 30 days of start (state verification preferred; special instructions for California RNs)

Professional Requirements

• Signed Trinity Confidentiality of Patient & Proprietary Information and System Security Agreement
• Motor Vehicle Report (MVR) dated within 30 days of start
• Specialty-specific competency testing completed within the last year
• Trinity Conflict of Interest Disclosure Form (signed)
• Criminal Background Check (federal, nationwide, county for last 7 years, including SSN verification, dated within 30 days)
• Education Verification (highest academic degree or diploma verified)
• EPLS (Excluded Parties List System) check dated within 30 days
• GSA/OIG exclusion check dated within 30 days
• Integrity and Audit Services Form (signed shortly before start)
• Joint Commission CORE Exam within the last year
• NSO (National Sex Offender) check dated within 30 days, including aliases and locations
• System Access Form completed and uploaded one week before start
• Skills Checklist (provided by agency for clinical contractors)

Personal Health and Testing

• COVID-19 Vaccine: Proof of booster by Sept 2024, or declination/exemption (declination not accepted in CA or IL)
• MMR: Proof of 2 vaccinations or IgG titer lab report
• Hepatitis B: Must have started vaccination series prior to start; titers not accepted
• Influenza Vaccine: Optional but recommended (proof or declination/exemption)
• 2 Step PPD or IGRA (Quantiferon/Tspot) with chest x-ray if positive (timing depends on state)
• Varicella: Proof of 1 vaccination or IgG titer lab report
• Physical/Health Statement: Exam within last year confirming fitness to work
• Fit Test (for respirator use)
• Trinity 10 Panel Drug Screen (9 panel plus Methadone, no THC) completed within 30 days of start, preferred within 14 days

This is a 13-week travel RN position in Labor and Delivery at Saint Alphonsus Medical Center Ontario, Oregon, with a critical need for night shift coverage. Candidates must have all required certifications, licenses, health screenings, and professional verifications current and compliant with Trinity Health onboarding standards.
