Description

Job Opportunity: Cath Lab Tech (Radiology / Cardiology)

Location: Findlay, Ohio
Employment Type: Travel/Contract
Contract Length: 13 weeks
Start Date: ASAP


Job Details

  • Position: Cath Lab Tech
  • Schedule: 4-10 hour shifts per week
  • Call Rotation: Every 4th weekend
  • Experience with Phillips Azurion and Merge Hemo for documentation is a bonus

Required Certifications

  • ACLS (Advanced Cardiac Life Support) – ARC, AHA, or US Department of Defense accepted
  • BLS (Basic Life Support) – ARC, AHA, or US Department of Defense accepted
  • PALS (Pediatric Advanced Life Support) – as applicable
  • NRP (Neonatal Resuscitation Procedure) – as required
  • TNCC (Trauma Nursing Core Course) – as required
  • State License – Primary Source Verification required
  • NURSYS Verification (for RNs)
  • Tech Certification Verification – refer to Allied Licensure Report; not required for CVOR Techs
  • Orientation Safety Manual Acknowledgement (HIPAA compliance)

Required Background Checks & Verifications

  • 7-year criminal background check within 30 days of start, covering all locations lived/worked in past 7 years
  • EPLS/GSA/SAM check within 30 days of start and updated for contract renewals
  • HHS/OIG check within 30 days of start and updated for contract renewals
  • Education Verification
  • Professional References (2 clinical or hospital evaluations accepted)
  • Performance Evaluation if worked over 1 year with the system
  • Internal Evaluation if worked over 2 years with the system
  • Complete Employment Application/Resume
  • Skills Checklist updated annually

Health & Immunization Requirements

  • Physical Exam: Initial and updated annually, attesting ability to meet physical demands
  • Negative TB Test (PPD, Quantiferon Gold, or T-spot) within 1 year of start; annual TB questionnaire required
  • Immunizations or Titers for:
    • Rubeola (Measles)
    • Mumps
    • Rubella
    • Hepatitis B (titer, vaccine series, or declination)
    • Varicella (chickenpox)
  • Drug Testing: Negative 10-panel drug screen within 30 days of start; not required annually while on assignment
  • Flu Vaccination or signed declination form required during flu season (10/1 – 3/31) with compliance deadline by 12/1
  • COVID-19 Vaccination: Fully vaccinated or approved medical/religious exemption with accommodations (masking, possible testing)
  • COVID-19 Exemption Attestation must be completed by agency if applicable

Facility-Specific Requirements

  • Contract Employee Checklist – must be signed and dated
  • Orientation to Work Environment – must be reviewed and signed
  • Badge Authorization Form – must be completed and signed
  • Confidentiality Agreement – must be reviewed and signed
  • System Access Form – must be completed prior to start
  • COVID-19 Vaccine Exemption Attestation (if applicable)
  • Flu Declination Form (if applicable)
  • Hepatitis B Declination Form (if applicable)
  • COVID-19 Unvaccinated Education Acknowledgement (if applicable)

Competency Exams

  • Medication/Pharmacology Competency Exam – as applicable, must meet passing score
  • Unit Competency Exam – required and must be updated annually
  • Core Parts I, II, III (for subcontractors) – HIPAA form or core testing acceptable

Additional Notes

  • Proof of I9 or Attestation required
  • Government-issued photo ID required and must be carried during hours of service
  • Residency Declaration Form required if working on a compact license
