Job Description
- This position is an extension of a current travel nurse contract.
- Cardiac-trained RN skills are required to support ongoing open heart orientation.
- The facility has a significant RN vacancy rate (~35.7%).
- Virtual nursing support is also part of the staffing model.
Required Certifications & Licenses
- BLS (Basic Life Support) – American Heart Association only, required for clinical positions.
- ACLS (Advanced Cardiovascular Life Support) – American Heart Association only.
- NIHSS (National Institutes of Health Stroke Scale) certification.
- Permanent RN License – Primary source verification required within 30 days of start; temporary licenses not accepted.
- Ohio Fingerprints – BCI&I check and FCRA-compliant national criminal background check required (different requirements if living in Ohio less than 5 years).
Required Health & Immunization Credentials
- COVID-19 Vaccine – Proof preferred but not required; medical or religious exemptions accepted with proper documentation.
- MMR (Measles, Mumps, Rubella) – Positive titers within last 15 years required; booster protocol applies if low titers.
- Varicella – Positive titers within last 15 years required; booster protocol applies if low titers.
- Hepatitis B – Proof of full series, partial series with declination, or positive titers within 15 years accepted.
- Tdap (Tetanus, Diphtheria, Pertussis) – Proof of vaccination within past 10 years required; no declinations accepted.
- Influenza Vaccine – Required during flu season (Oct-Mar) or approved medical/religious exemption.
- 2-Step PPD or IGRA (Quantiferon/Tspot) – Negative test within last 6 months required; if positive, clear chest x-ray and TB questionnaire needed.
- Fit Test – Completed within 1 year of start, specific mask types accepted.
- Physical/Health Statement – Pre-employment physical exam within 1 year stating ability to work without restrictions.
Professional & Compliance Requirements
- UHS I-9 Attestation – Proof of eligibility to work in the U.S.
- Social Security Trace – If not included in background check, to be completed within 1 year of start.
- National Criminal Background Check – Including SSN trace, county searches, and NSO; must be within 6 months of start.
- OIG Exclusion Search – Completed within 6 months prior to start; only required once upon initial hire.
- SAM/EPLS/GSA/OFAC Clearance – Completed within 6 months prior to start; only required once upon initial hire.
- Drug Screen – 10-panel drug screen within 60 days of start; instant drug screens not accepted.
- Education Verification – Highest level of education verified; international education requires equivalency documentation.
- Employment Verification – Verification of employment within last 5 years through third-party; attestations not allowed.
- Competency Exams – Must score at least 80% on relevant department exams within 1 year prior to start.
- Core Competencies – HIPAA, Joint Commission, OSHA topics completed within 1 year prior to start; 80% passing score required.
- Skills Checklist – Clinical skill-specific checklist completed within 1 year prior to start (initial hire only).
- UHS Module Completion Certificate – Safety and compliance training modules must be completed prior to clearance.
- Badge Photo – Professional headshot required.
- Driver’s License/State ID – Non-expired ID required for submission.
- Submission Packet – Resume including education and work history, 2 professional references within last 2 years, and completed job application required.
