Job Opportunity: MRI Technician (Night Shift)

Location: Westerville, Ohio
Employment Type: Long-Term Assignment (LTA)
Contract Length: 13 weeks

Work Schedule

• Night shift: 7:00 PM – 7:30 AM
• Workdays: Either Thursday/Friday/Saturday or Friday/Saturday/Sunday

Required Credentials & Onboarding Requirements

Certifications & Licenses

• BLS (Basic Life Support): American Heart Association certification required for all direct patient care personnel. Must provide front and back images of the card.
• License/Certificate Verification: Must be dated within 30 days of start. Online verification page with machine-generated date required.

Health & Drug Screening

• Fit Test: Required prior to start.
• Trinity 10 Panel Drug Screen: Completed within 30 days of start (preferred within 14 days). Must include specific substances (no THC). Results must be finalized with MRO signature before start.
• COVID-19 Vaccine: Proof of vaccine or approved declination/exemption required. Booster valid through September 2025; declination not accepted in CA or IL.
• MMR: Proof of 2 vaccinations or lab IgG titer required.
• Hepatitis B: Must have started vaccination series prior to start; titers not accepted.
• Influenza Vaccine: Proof of vaccine, declination, or exemption required for current flu season. Declination not accepted in CA or IL.
• 2 Step PPD OR IGRA (Quantiferon/Tspot): TB testing required; positive TB requires chest x-ray within specified timeframes.
• Varicella: Proof of vaccination or lab IgG titer required.
• Physical/Health Statement: Documentation from exam within last year confirming fitness to work.

Professional & Compliance Documents

• Trinity Code of Conduct Form: Signed form required.
• Trinity I9: Standard employment eligibility verification completed by agency.
• Confidentiality & Security Agreement: Signed agreement regarding patient and proprietary information required.
• Integrity and Audit Services Form: Signed shortly before start date.
• Conflict of Interest Disclosure Form: Signed form required.
• Criminal Background Check: Federal, nationwide, and county checks covering last 7 years, including SSN verification, dated within 30 days of start.
• Education Verification: Highest academic degree or diploma/GED verified by third party screening.
• EPLS (Excluded Parties List System): Report dated within 30 days required.
• GSA/OIG Exclusion Check: Report dated within 30 days required.
• Joint Commission CORE Exam: Completed within the last year.
• NSO (National Sex Offender) Check: Dated within 30 days of start, including all aliases and locations. Additional statewide check for Michigan.
• System Access Form: Must be completed and uploaded one week before start.

Competency & Skills

• Specialty Specific Competency Testing: Specialty exam must be completed within the last year.
• Skills Checklist: Provided by agency for clinical contractors; required for onboarding.

Summary
