Job Opportunity: Respiratory Therapist

Location: Ontario, Oregon
Employment Type: Long-Term Assignment (LTA)

Required Credentials and Onboarding Requirements

Licenses & Certifications

• BLS (Basic Life Support):

  • American Heart Association certification required for all direct patient care personnel.
  • Must provide scanned copy of both front and back of card with expiration date.
• License/Certificate Verification:

  • Must be dated within 30 days of start.
  • Attach scanned online verification with machine-generated date within past 30 days.
• Specialty Specific Competency Testing:

  • Specialty exam must be completed within the last year.

Personal Health & Immunizations

• Fit Test
• Trinity 10 Panel Drug Screen:

  • Completed within 30 days of start (preferred within 14 days).
  • Must include: Cocaine, Amphetamines, Opiates, 6-Monoacetylmorphine, Propoxyphene, PCP, Barbiturates, Benzodiazepines, Methaqualone, Methadone.
  • No THC allowed.
• COVID-19 Vaccine:

  • Proof of booster by September 2025, or declination/exemption with agency letterhead.
  • Declination not accepted for CA or IL.
• MMR:

  • Proof of 2 vaccinations with dates or IgG titer lab report.
• Hepatitis B:

  • Must have started vaccination series prior to start (1st shot before start, 2nd shot 30 days later, 3rd shot 6 months later).
  • Titers not accepted.
• Influenza Vaccine:

  • Proof of vaccine, declination, or exemption with agency letterhead.
  • Declination not accepted for CA or IL.
  • Valid for flu season 10/1/2024 - 4/30/2025 and 10/1/2025 - 4/30/2026.
• 2 Step PPD OR IGRA (Quantiferon/Tspot):

  • Positive TB requires chest x-ray within last year for MI, IN, CT, MA; within 5 years for other states.
• Varicella:

  • Proof of 1 vaccination with date or IgG titer lab report.
• Physical/Health Statement:

  • Documentation from exam within last year confirming fitness to work.

Professional & Compliance Requirements

• Trinity Code of Conduct Form: Signed form required.
• Trinity I9: Standard I9 completed by agency.
• Confidentiality & Security Agreement: Signed agreement regarding patient and proprietary information.
• Conflict of Interest Disclosure Form: Signed form required.
• Criminal Background Check:

  • Federal, nationwide, county checks for last 7 years.
  • SSN verification included.
  • Must be dated within 30 days of start.
• Education Verification:

  • Highest academic degree or diploma/GED verified by third party screening.
• EPLS (Excluded Parties List System):

  • Scanned report dated within past 30 days.
• GSA/OIG Exclusion Check:

  • Scanned report dated within past 30 days.
• Integrity and Audit Services Form: Signed shortly before start.
• Joint Commission Testing: CORE exam within last year.
• NSO (National Sex Offender) Check:

  • Dated within 30 days of start.
  • Includes all aliases and locations.
• System Access Form: Completed and uploaded one week before start.

Skills & Competency

• Trinity Skills Checklist: Provided by agency for clinical contractors.

This position requires thorough credentialing and compliance with health and professional standards prior to start.
