Job Description

Outpatient Physical Therapist position providing care in an outpatient setting.

Housing

• Shared housing available (no guests or pets allowed)
• Must have own transportation
• Housing agreement required only if moving into shared housing

Required Credentials & Compliance

Professional Credentials

• Resume including past 7 years of work experience (with city & state) - required for submission
• Two recent references (minimum one supervisor or above in skillset) - required for submission
• Skills checklist - required for submission
• Confirmation of Assignment/POI form - required upon acceptance and for extensions
• Criminal background check within 45 days prior to start and updated if >6 month break
• Massachusetts CORI background check within 30 days prior to start and updated if >6 month break
• Education verification of highest level achieved
• Social Security Number trace (addresses/aliases for past 7 years)
• National sex offender registry check within 30 days of start and updated annually
• Exclusion checks (OIG, GSA, MassHealth, NH Medicaid) within 30 days of start and annually

Licenses & Certifications

• Professional license verification with primary source verification within 30 days of start (MA Conditional Licenses not accepted)
• Basic Life Support (BLS) certification (AHA, Red Cross, CHSF, or Military accepted; AHA preferred)
• Respirator Medical Clearance Attestation if fit test required

Personal Health Requirements

• Flu vaccine required during flu season (Sept 15 - April 30); no exemptions allowed
• Measles, Mumps, Rubella (MMR) vaccination or proof of immunity
• Tuberculosis (TB) test documentation within 3 months of start (skin test or blood test) and updated if >6 month break
• Physician health statement signed by MD/DO/NP/PA within 365 days of start and updated if >6 month break
• Hepatitis B vaccination or immunity strongly preferred (optional)
• Tdap vaccine within 10 years preferred (optional)
• Positive PPD requires negative chest X-ray documentation (optional)
• TB symptom survey if positive PPD (optional)
