Location: Flagstaff, Arizona

Compliance & Documentation Requirements
- Identification: Valid Driver’s License or State Photo ID (passport alone not accepted)
- Background Checks: 7-year background check within 30 days of start, including SSN trace, criminal search, sex offender search
- Certifications Verification: Must have current legal name on documentation; ARRT certification required for Cath Lab Tech
- State License: AZ State Certified Technologist in Computed Tomography (if applicable)
- Competency Exams: Medication test/pharmacology and competency exams required with minimum passing scores
- Health Documents:
- Physical exam within 1 year confirming ability to perform essential job functions
- Fit mask test (N95) within 1 year, updated annually (PAPR not an option except in limited cases)
- Immunizations and titers: Tdap, Measles, Mumps, Rubella, Hepatitis B, Varicella
- Negative TB test or chest x-ray within 1 year, updated annually
- Drug testing (10 panel) within 30 days of start, updated annually
- Seasonal flu vaccination or signed declination form required
- COVID-19 vaccination or signed declination form required
