Job Description

• Specialty: Registered Nurse in Critical Care
• Schedule: Every Other Week (EOW) / Every Other Holiday (EOH)

Required Credentials & Certifications

• Licenses:

  • Valid nursing license with verification dated within 30 days of start
• Certifications:

  • BLS (American Heart Association) – front and back of card required
  • ACLS (American Heart Association) – required for CCU, ED, PACU, MICU, SICU; front and back of card required
  • NIHSS
• Competency Testing:

  • Specialty-specific competency exam completed within the last year
  • Trinity Core Exam within the last year
• Other Professional Requirements:

  • Signed Trinity Code of Conduct Form
  • Trinity I9 form completed by agency
  • Confidentiality and Information System Security Agreement signed
  • Integrity and Audit Services Form signed shortly before start
  • Non-Employee Upload Form fully completed
  • Conflict of Interest Disclosure Form signed
  • Criminal Background Check (Federal, Nationwide, County for last 7 years) dated within 30 days of start, including SSN verification and employment verification
  • Education Verification (highest level of academic education verified)
  • EPLS (Excluded Parties List System) report dated within 30 days
  • OIG (Office of Inspector General) exclusion check dated within 30 days
  • NSO (National Sex Offender) check dated within 30 days, including all aliases
  • System Access Form completed and uploaded one week before start
  • Skills Checklist provided by agency for clinical contractors
  • Child and Adult Dependent Abuse Training (Iowa only)
  • Iowa Sing Report (Criminal History, Sex Offender, Dependent Adult & Child Abuse Registries) dated within 30 days prior to start
  • OFAC (Office of Foreign Assets Control) sanctions check completed within 30 days of start

Health & Immunization Requirements

• Fit Test
• Trinity 10 Panel Drug Screen (completed within 30 days of start, preferred within 14 days; must include specific substances; no THC)
• COVID-19 Vaccine: Proof of booster by September 2025, or declination/exemption with agency letterhead (declination not accepted for CA or IL)
• MMR: Proof of 2 vaccinations with dates or IgG titer lab report
• Hepatitis B: Must have started vaccination series prior to start; titers not accepted
• Influenza Vaccine: Proof of vaccine, declination, or exemption with agency letterhead; flu season dates apply; declination not accepted for CA or IL
• 2 Step PPD OR IGRA (Quantiferon/Tspot): Chest x-ray required if positive TB, with timing depending on state
• Varicella: Proof of 1 vaccination with date or IgG titer lab report
• Physical/Health Statement: Documentation from exam within last year confirming fitness to work
• Tdap: Current, non-expired vaccine required; declinations accepted if no pediatric interaction
