Registered Nurse (Home Health) - Oregon (Salem Area)
Employment Type: Travel/Contract
Contract Length: 13 weeks
Location: South of Salem, including Albany and Corvallis, Oregon
Schedule: Monday - Friday, 8 AM - 5 PM
Weekend Rotation: On-call 1 weekend every 4th week
On-call Requirement: Yes, 1 weekend on call every 4th week
Average Patients per Day: 5-8
Weekly Productivity Expectation: 30 units
Estimated Weekly Travel: Approximately 250 miles
Territory Coverage: Must be willing to travel up to 75 miles from home clinic
Dress Code: Scrubs
Job Description
- Provide, coordinate, and direct home nursing care according to physician orders and agency policies.
- Supervise and evaluate Home Health Aides and Licensed Practical Nurses/Licensed Vocational Nurses.
- Conduct home visits and attend periodic office meetings.
- Types of cases include Start of Care, Wound Care, Follow-up Visits, etc.
- Use HomeCare HomeBase (HCHB) EMR system and be proficient in OASIS documentation.
- This is a backfill position.
Requirements
- Experience: Minimum 1 full year of recent Adult Home Health or Hospice experience (depending on specialty).
- First-timers: Not accepted; must have recent specialty experience.
- Certifications: Current CPR and BLS required.
- Licensure: Must have an active RN license in hand; pending licenses are not accepted.
- Transportation: Reliable transportation, current driver's license, and agency-required liability insurance required.
- EMR Experience: Proficiency with HomeCare HomeBase (HCHB) within the last 6 months required; retraining may be required if last used over 6 months ago.
- Other: Must be willing to discuss weekend and on-call requirements during interview.
Additional Information
- Points System: Contractor assigned 5-10 patients per day equating to roughly 6-8 points; 30 points expected weekly.
- Local Candidates: Accepted at the same rate.
- Mileage Reimbursement: Paid at IRS standard rate for travel between patient homes (commute to/from first and last patient not billable).
- Shift Patterns: Some positions may follow a Baylor Shift schedule (4x10s, Friday-Monday), but most are standard 5x8, Monday-Friday.
Compliance and Documentation Requirements
- Must complete and maintain all required background checks, including but not limited to:
- 7-year standard background check (including SS trace, county, nationwide, sex offender, and state-specific searches).
- EPLS/GSA/SAM and HHS/OIG screenings.
- Fingerprinting as required by state regulations.
- Education verification for nursing degree.
- Drug screening (10-panel).
- Immunizations and health documents including:
- Physical exam within 12 months.
- Negative TB test or chest X-ray within 12 months.
- Proof of immunity or vaccination for Measles, Mumps, Rubella, Varicella, Hepatitis B.
- Seasonal flu and COVID-19 vaccination or documented declination.
- Must review and sign facility-specific compliance documents including:
- Code of Conduct, Confidentiality Forms, HIPAA and Privacy Policies.
- Home Health Core Policy and Scenarios.
- COVID-19 Policy and Attestation.
- Employee Responsibility Agreement.
- Must have personal or auto insurance policy under candidate’s name or list candidate as a driver.
Summary
This is a 13-week travel contract RN position in home health nursing in the Salem, Oregon area. The role requires recent home health or hospice experience, proficiency with specific EMR systems, and the ability to travel within a 75-mile radius.
