Physical Therapist Assistant (Travel/Contract)
Location: South Haven, Michigan

- Shift: 1st/Days
- Hours: 40 hours/week (0700-1815 Monday-Thursday, 0700-1645 Friday)
- Guaranteed Hours: 40 hours/week
- No call, weekend, or float requirements
Job Description
Clinical Duties
- Work under the guidance of a licensed Physical Therapist
- Follow the plan of care and goals established by the Physical Therapist
- Provide direct patient care including physical, electrical, mechanical, and heat treatments
- Conduct individual and/or group Physical Therapy activities
- Educate family and community
- Participate in interdisciplinary team discussions and hospital rounds
- Assist in program development and serve as a resource in areas of expertise
- Participate in student programs and rotations
- Provide in-services within Rehabilitation Services
- May cross-train and provide coverage in other Rehabilitation departments as needed
Administrative Duties
- Document in electronic health records per compliance timelines
- Communicate any limitations preventing timely documentation to leadership
- Maintain accurate, legible, and complete records for each patient
- Serve on committees and task forces
- Participate in process improvement initiatives
- Demonstrate effective time management skills
- Supervise Rehab Clinical Assistants, PTA students, and volunteers
Physical Requirements
- Considerable standing or walking
- Regular lifting of light-weight objects (20 pounds or less)
- Assisting with heavier lifting and moving of patients
- Some fatiguing physical motions and minor straining positions
Required Certifications & Competencies
- BLS (Basic Life Support) - AHA only
- ACLS (Advanced Cardiac Life Support) - AHA only (as applicable)
- PALS (Pediatric Advanced Life Support) - AHA only (as applicable)
- NRP (Neonatal Resuscitation Procedure) - AAP or American Medical Learning Center (as applicable)
- TNCC (Trauma Nursing Core Course) (as applicable)
- EFM (Electronic Fetal Monitoring) (as applicable)
- NIH Stroke Certification (AHA, APEX, Bluecloud) - must be updated annually (as applicable)
- CPI (Crisis Prevention Institute) (as applicable)
- Medication Test/Pharmacology Competency Exam - completed within 1 year prior to assignment with passing score ≥80%
- Competency Exam in specialty - completed within 1 year prior to assignment with passing score ≥80%
- Color Blind Testing Online (required for laboratory settings only)
- Orientation Safety Manual Acknowledgement
Licensure & Verification
- State License primary source online verification required
- Education verification (copy of highest degree or diploma)
- National Tech Certification as applicable (e.g., ASCP, AMT, MLS, MT, MLT for Med Techs)
- Skills checklist updated annually (if applicable)
Background & Compliance Requirements
- Criminal background check within 45 days of start, including:
- SSN Trace
- National Criminal Search
- National Sex Offender Search
- Statewide searches
- Education verification
- Motor Vehicle Record (MVR) search for Home Health
- Monthly EPLS/GSA/SAM and HHS/OIG checks
- MI LARA fingerprinting required within 45 days of start for certain positions and locations
- Michigan Central Registry Clearance required for pediatric, RRT, L&D, and Echo Tech Cardiac Sonographer roles
- Drug testing: 10-panel including Marijuana/THC within 45 days of start
- References: One professional reference from supervisor/manager required
- Complete employment application/resume with full 7 years work history, no gaps
Health & Immunization Requirements
- Physical exam within 1 year of start (not required for non-clinical roles)
- Fit mask test (Bronson requires Honeywell DC365N95HC) - annual expiration for Aya candidates
- Tetanus/Diphtheria/Tdap (must include pertussis) - no declinations accepted
- Negative TB test (PPD or Q gold/Tspot) within 45 days of start; if positive, chest x-ray and TB questionnaire required
- TB questionnaire updated annually
- Rubeola (Measles), Mumps, Rubella: 2 vaccines or positive titer, no exemptions accepted
- Hepatitis B: titer, series, or facility declination required
- Varicella: 2 vaccines or positive titer, no exemptions accepted
- Seasonal Flu Vaccination or approved medical/spiritual exemption required for candidates starting after 11/1; proof or exemption must be submitted by 12/1
Facility Specific Requirements
- Review and sign Code of Conduct and Appearance Standards Policy prior to start
- Hepatitis B declination form if declining vaccine or titer
- Fit test appointment must be completed onsite before or on first day
- Medical and vaccine exemption forms require approval and must be submitted in advance
- Dosimetry History Request Form required for Radiology specialties
